• Please submit this form at least 2 days in advance of your desired service time. We will contact you by phone and/or email (if provided) to either confirm your requested appointment, or discuss an alternative if your day/time is not available or requested service(s) cannot be performed within the time requirements you specified.

• To drop off your vehicle outside our normal business hours (Mon-Fri 8 am – 6 pm), you may use our Early/Late Drop-Off box. If so, please check the item for this in the form below.

• For a service, repair or problem diagnostic not appearing in the Services checklist, put specifics in the Description field immediately following. Be aware that diagnostic time may be difficult to estimate in advance and repairs requiring parts not in stock may not be completed the same day.

• If we discover additional items or problems during requested service which we reasonably believe should be addressed immediately, we will first contact you to advise and obtain authorization before such additional work.
